Groomer Barn/Club House Donation Form

I make this donation with the understanding that any excess funds obtained will be placed in the fund established for the construction of a clubhouse. I understand that my name will be listed on a plaque posted inside the groomer barn and club house acknowledging cash donations and donations of labor and materials. 

If you are willing to donate labor, materials, etc. during the construction of the barn, please list below skills you can help us with (i.e. plumbing, electrical, carpentry, etc.), or the materials you may wish to donate, and a phone number where you can be reached when construction begins.
